Stormont-Vail HealthCare, Inc. (“Stormont-Vail”) is a Kansas nonprofit 501 (c)(3) corporation. Stormont-Vail operates a tertiary acute care hospital (the "Medical Center") located in Topeka, Kansas. On September 1, 1995, Stormont-Vail purchased Cotton-O'Neil Clinic, Inc. (“COCI”). This transaction involved the acquisition of 72 physicians in a multispecialty clinic with seven locations in Topeka and northeast Kansas. On December 1, 1996, Stormont-Vail HealthCare acquired Pediatrics, P.A. (“PediatricCare”). This practice was a six physician pediatric clinic with three locations in Topeka. The regional Cotton-O’Neil and Stormont-Vail clinics in the communities of northeast Kansas continue to operate under their original names. The physicians of Cotton-O’Neil Clinic, PediatricCare, and the regional clinics now constitute the Medical Services Division of Stormont-Vail HealthCare and are functioning as an integrated health care delivery system.

CARBONDALE
Cotton-O'Neil Clinic-Carbondale - Carbondale is approximately twenty miles south of Topeka on Highway 75 and Osage County is considered to be medically under served. The Cotton-O’Neil Clinic established the Carbondale Clinic in 1984 and another clinic in Lyndon, Kansas (The Lyndon Clinic) in 1988. These clinics were combined and expanded in 1993 to form the current Osage County Clinic. There are three primary care doctors at the Osage County Clinic. David Penn, MD, is a family practitioner who provides obstetrical care. He is on the medical staff at both Stormont-Vail Regional Medical Center and St. Francis Hospital in Topeka. EMPORIA
Medical Arts Clinic of Emporia and Lebo In 1994, Cotton-O’Neil Clinic continued regional expansion when the doctors of Medical Arts Clinic, P.A. of Emporia and Lebo merged with Cotton-O’Neil Clinic. Another Emporia clinic (Family Physicians of Emporia, P.A.) merged into that practice in June of 1995. There are now ten physicians in this group. Family practitioners include Todd Detwiler, MD, Pam Harrison, MD, Rob Hutchison, MD, Robert Stewart, MD, and Sam Tovar, MD. Internists include Cy Anderson, MD, and Scott Smiley, MD. Tom Hicks, MD is a general surgeon. David Kemp, MD provides Ob/GYN services and Dr. Merle Milburn is a pediatrician. This group is located in Newman’s Medical Plaza which is a beautiful new office facility adjacent to Newman County Hospital. Dr. Hutchison practices in a satellite office in Lebo, Kansas. Both Emporia in Lyon County and Lebo in Coffee County are considered medically under served. LAWRENCE
Cotton O-Neil Clinic-Lawrence Scott Solcher, MD and Cory Sise, MD joined Bob Porter, MD as Cotton-O’Neil’s second and third nephrologists. Dr. Solcher has primary office space located at the Lawrence Health Plaza in Lawrence. His office is directly across the hallway from the Dialysis Center at Lawrence Health Plaza. MERIDEN
Cotton-O'Neil Clinic-Meriden Cotton-O’Neil ClinicMeriden opened in September, 1997. Meriden is located on Highway 4 in Jefferson County approximately ten miles northeast of Topeka. This office was designed for two physicians and has on-site x-ray and mammography. OSAGE CITY
Osage Medical Clinic The Osage Medical Clinic is located at 608 Holiday in Osage City. This clinic was originally founded by brothers, Dr. Paul Adams and Dr. Dwight Adams. Dr. James Seeman joined the practice prior to their retirement. Dr. Seeman practices out-patient family medicine and is the only physician in Osage City. OSKALOOSA
Cotton-O'Neil ClinicOskaloosa Oskaloosa is located in the eastern part of Jefferson County approximately 25 miles northeast of Topeka. Oskaloosa is the county seat of Jefferson County. Cotton-O’Neil Clinic acquired the practice from Jefferson County Memorial Hospital in 1999. Darrin Cox, PA provides primary care services at the facility and Laurie Conway, MD joined the practice in 2001 as an IM/Peds physician. Dr. Conway is also the supervising physician for Darrin Cox. ROSSVILLE
Rossville Clinic The Rossville Clinic became a satellite of the Cotton-O’Neil Clinic in 1992. Pattie Klassen, MD joined the Rossville Clinic after Dr. Myron Leinwetter, who had been in Rossville at 423 Main since November 28, 1989, left to pursue missionary work with his family. Dr. Klassen practices out-patient family medicine. Rossville is in the western part of Shawnee County on Highway 24. Cotton-O'Neil Clinic The Cotton-O'Neil Clinic was formed in 1955 as a professional association originally known as Internal Medicine, P.A. The founding fathers were Drs. Robert Cotton and Robert O'Neil. Expansion began in 1968 when the doctors decided Topeka needed more subspecialty internists. The group built the 901 Garfield building in 1974 and added an addition in 1979. The building was renamed the Cotton Building in 1987. A second medical office building at 823 S.W. Mulvane (The O'Neil Building) opened in 1987.
Cotton-O'Neil ClinicUrish Road This clinic is located at 29th and Urish Road in southwest Topeka. This beautiful facility was designed for six physicians and also has on-site x-ray and mammography services. Linda Overholser, MD is an internist. George Evans, MD is a family practitioner and provides obstetrical care and William Lentz provides his services as a family practitioner at this location as well. Cotton-O'Neil ClinicCroco Cotton-O’Neil opened a new facility at 29th and Croco Road in southeast Topeka in August of 1998. Mark Thomas, MD has practiced internal medicine in Topeka since 1995 and relocated to this facility from Highland Park Clinic. Drs. Jon and Melissa Schroeder, both internists, joined Dr. Thomas in 2003. They currently share a full time practice. Kent Haverkamp, MD is boarded in both internal medicine and pediatrics and relocated from Carbondale to this facility in August of 98. He was joined on in 1998 by Dr. Ann Sachs who is also board certified in both internal medicine and pediatrics. Click here for more information.
Cotton-O’Neil Heart Center The Cotton-O’Neil Heart Center is located near Stormont-Vail at 929 SW Mulvane. The Cotton-O’Neil cardiologists and their staff members assisted in designing this building. Physician offices and exam rooms are conveniently located on the first floor. Diagnostic testing services including sonography, echocardiography and nuclear imaging are located on the second floor. A suite in the lower level is specifically designed for providing ECP therapy. Another feature of the building is a kitchen designed for live cooking demonstrations. The highlight of the building is the cardiac rehab center which has a 360 degree elevated walking track located on a balcony overlooking the floor space of the rehab center. Cotton-O’Neil Digestive Health Center
The Cotton-O’Neil Digestive Health Center opened on January 4, 2004 and is located at 720 SW Lane. The gastroenterologists assisted in designing this building. Hospitalists Cotton-O’Neil Clinic began a hospitalist service in December of 1998. The hospitalists are board certified internists dedicated to providing inpatient care at Stormont-Vail Hospital. The hospitalists are available 24-hours a day/7-days a week and accept referrals from physicians throughout northeast Kansas. PediatricCare Pediatrics, P.A. was acquired by Stormont-Vail HealthCare on December 1, 1996. The name of the practice was then changed to PediatricCare. The physicians of PediatricCare include Steven Crouch, MD, Dan Kelly, MD, Brandan Kennedy, MD, Heather Rainwater, MD, Harold Parr, MD, F.E. Patrick, MD, and Daniel Reynolds, MD. WAMEGO
Cotton-O'Neil ClinicWamego Cotton-O’Neil Clinic opened a new facility in Wamego in December of 1999. This building is located on Highway 24 on the west end of Wamego. Prior to the construction of this building, the five physicians in Wamego had been practicing in three different locations. The physicians at Cotton-O’Neil Clinic Wamego are all family practitioners and include Jeff Atwood, MD, Lou Borgendale, MD, Robert Tackett, MD, Michael Malin, MD and new to the clinic in 2003, Dr. Linda Skiles. These physicians all live in Wamego and are on the medical staff at Wamego City Hospital. Wamego, Kansas is located in Pottawatomie County and is considered medically underserved. Income and Benefits
Physician Income. The initial employment contract provides a guaranteed salary with a bonus opportunity based on productivity and quality guidelines. Following successful completion of the initial contract, the physician is asked to sign a long-term employment agreement and to become an equal shareholder in Kansas Physicians’ Association.
Shareholder physicians within the medical services division are paid a salary that is primarily based on productivity. At this time, productivity is measured using the Medicare relative value unit (RVU) which was developed as the foundation of RBRVS. Physicians are provided monthly reports showing them their RVU production.
Retirement Benefits. As an employed physician, there is immediate participation in the Stormont-Vail HealthCare retirement plan. This retirement plan is a defined benefit plan providing lifetime benefits based on formulas that take into account years of service and average salary.
Stormont-Vail also offers a 403b plan which provides the opportunity to defer up to $13,000 of pre-tax income into a retirement fund. These monies may be invested in any or all of five mutual fund families including Fidelity, Twentieth Century, and Security Benefit Group, T. Rowe Price and Vanguard. For the first 4 percent of pay you save, Stormont-Vail will match 50 cents of every dollar. The maximum match from Stormont-Vail is $1,000 per year.
Professional Liabilities. All professional liability premiums are paid in full. The current malpractice carrier is Zurich. The physicians are insured with 200,000/600,000 basic coverage from Zurich and 800,000/2,400,000 from the stabilization fund.
Term Life Insurance. SVHC pays the full premium for term life insurance. Coverage is 150 percent of base salary capped at $550,000. Additional life insurance for physician and/or dependents may be purchased by physician.
Health Insurance. Health insurance benefits are offered through a group policy using Century Health Network. SVHC pays 80 percent of the premium and the physician pays 20 percent. The physician portion may be paid with pre-tax income.
Disability Insurance. Physicians are eligible to participate in a group disability insurance policy. Physicians have the option of purchasing $2,000, $4,000 or $6,000 disability benefits per month. The rates are typically lower than similar disability insurance purchased as an individual. Furthermore, all physicians are eligible regardless of past medical history or pre-existing conditions. This insurance is purchased by the physician rather than the corporation. This allows the disability income to be non-taxable.
Flexible Spending Benefits. There are two opportunities for flexible spending accounts. The physician may elect to defer up to $5,000 in pre-tax income to be used to pay for health needs not covered by health insurance. A similar flexible spending account (up to $5,000) is available for child care expenses.

Vacation Benefits. Physician is entitled to six weeks of vacation to be used for vacation or CME.
