Youth Program (13-17 years of age)                       

General Information
Youth volunteers may participate in one or both of the summer or academic school year programs. For questions, call (785) 354-6095.

Summer Program
Applications will be accepted Feb. 1 through May 10, 2011. After submitting your application online and collecting the additional information required (see application process) you will need to call Volunteer Services, 354-6095 to set up an interview one of the following dates: 

  • Sat., April 30 – 9 a.m–3 p.m.
    or
  • Wed., May 11 – 3–6 p.m.

A commitment of at least one shift per week for 8 out of the 11-week summer program is required. The 2011 summer program begins June 1 and ends Aug. 12.

Academic School Year Program
Applications may be submitted Sept. 1 through Feb. 1. (After Feb. 1, applications will be considered for the summer program). After submitting your application online and collecting the additional information required (see application process) you will need to call Volunteer Services, 354-6095 to set up an interview.

Qualifications

  • Must be at least 13 years of age
  • Maintain a “C” average during the course of the school year
  • Complete the application process and participate in mandatory orientation and training sessions, as required
  • Permission from a parent or guardian to participate, if under 18 years of age

Other Requirements

  • Purchase a volunteer polo shirt
  • Complete a minimum of 32 hours during the summer program or 50 hours during the academic school year
  • Receive a two-part Tuberculin Skin Testing (PPD) provided by hospital
  • Complete the health review (with parent/guardian signature)
  • Provide recommendation from teacher/guidance counselor
  • Receive a flu shot provided by hospital, if volunteering for the academic school year
  • Documentation of MMR, varicella and any other immunizations

Step Two
Information Gathering

Before calling to set up an interview you will need to collect the following information.  Please do not call for an interview until you have collected the following:
BRING ALL OF THE COMPLETED DOCUMENTATION TO THE INTERVIEW

  1. Copy of immunization record. This should include two (2) measles, mumps, rubella immunizations (MMR) and varicella (chicken pox) immunizations. (If applicant has had chicken pox, provide that date at the interview).
  2. Copy of recent grade report (can be last quarter report if current one is unavailable)
  3. Review volunteer opportunities, print and complete placement information sheet. Complete placement information sheet for whichever program you are applying for.

    2. Summer placement information sheet
  4. Pre-Volunteer Health Review  (Print out and complete.  Be sure to include parent/guardian consent).
  5. Print the Teacher/Guidance Counselor Reference Form. Complete top portion of the form and give to reference. Upon completion by the teacher or counselor, they will fax the form to Volunteer Services. (Fax# 785-354-5812)
  6. Review Uniform Requirements.  You will be asked to provide your uniform size at the interview so that, if accepted, uniform will be ready by your first day of volunteering. Cost is $20 and checks are payable to Stormont-Vail HealthCare.
  7. Print the Seasonal Influenza Consent/Declination Form and complete the information (front and back).  This is required for the academic school year program ONLY.

Step Three
Call (785) 354-6095 (Monday through Friday – 7:30 a.m. to 4:30 p.m.) for interview. Interview times for the 2011 summer program are:

  • Sat., April 30 – 9 a.m. – 3 p.m. (Keep in mind that you will need to return on May 2 or 3 between 7:30 am – 5:00pm to have your TB test read)  
    –or–
  • Wed., May 11 – 3 – 6 p.m. (Keep in mind that you will need to return on May 13 between 7:30am – 5:00pm to have your TB test read)

Step Four
At interview, you will also receive the first part of the tuberculin skin test – a mandatory requirement.

Again, ALL PAPERWORK NEEDS TO BE COMPLETE AND BROUGHT TO INTERVIEW.

We encourage that a parent/guardian accompany applicant as we may have questions about scheduling, consent forms, etc.

Your 2nd TB test will be scheduled at the interview so have your calendar available to schedule this.

If accepted, volunteer placement will also be provided at the conclusion of the interview. Details about the the mandatory orientation will be given at that time.

Step Five
Mandatory Orientation
All participants are required to attend one of the orientation sessions scheduled. They will also receive their uniform and have their picture taken for their photo ID. Orientation times are:

  • Sat., May 21 – 9:30 a.m.- Noon
    –or–
  • Thurs., May 26 – 1-3:30 p.m.
  • Location:  Pozez Education Center, 2nd floor

Step Six
Preparing for First Day
At the end of May, summer volunteers will receive their summer schedule and job description of the area(s) they will be volunteering by e-mail.

Step Seven
First Day
Volunteers will arrive on their first day in Volunteer Services, receive ID badge, sign final paperwork and get introduced to their volunteer area.
