Primary care providers are your home-base physicians. While you might see a specialist, such as a cardiologist for heart disease, your primary care provider is who you come back to at the end of the day, and who takes care of your body and mind.
What is Family and Internal Medicine?
Family physicians and internists are two types of primary care providers. Both are very similar – they are your go-tos for everything from everyday illnesses, to annual physicals, to managing common health conditions (such as diabetes).
Internists only treat adults, while family physicians treat any member of the family, regardless of age. Family physicians are often able to provide certain specialty services, such as obstetrics and gynecology.
Seeing either type of primary care provider on a regular basis is key to maintaining good health. Your provider can look for early signs of disease, refer you to specialists and help you keep your overall health and well-being in check.
Your provider will work with you to identify your needs and goals and to develop a customized plan to keep you healthy and strong.
Services We Provide
Primary care providers offer a number of services, including:
- Annual physicals, sports physicals, and back-to-school exams
- Preventive care and screenings (e.g., mammograms, bone density tests)
- Healthy lifestyle education, like information about diet or vaccines
- Diagnosis and treatment for common medical conditions, such as diabetes or high blood pressure
- Diagnosis and treatment for minor illnesses, such as the flu or strep throat
- Education about what to expect at each stage of life when you have a chronic illness
- Referrals for specialists, if needed
Intensive Primary Care
When patients have chronic medical conditions or psychosocial issues, and are constantly using health care services, they may be good candidates for intensive primary care.
Intensive primary care aims to cut costs and reduce the amount of times that the patients need to be cared for in the hospital.
If you need primary intensive care, you will see Dr. Pampolina at the Intensive Primary Care Clinic. Dr. Pampolina will coordinate with a team to make sure that every one of your chronic medical and social needs are met, and that each of your providers is coordinating with one another.
Patients are referred to primary intensive care by their current primary care provider or specialist.

Cotton O’Neil Wellness Clinic
Eligible Medicare patients can receive a preventive wellness visit at their primary care provider’s office. The visit is not in place of an annual physical – it’s focused primarily on prevention screenings and keeping your health record up-to-date.
At your wellness visit, you will work with a registered nurse to create an individualized prevention plan. The plan may include referrals to other preventive services, health education classes, or visits with specialists.
Each wellness visit includes:
- Routine measurements, such as height, weight, and blood pressure
- Review of medical and family history
- Establishing a list of current providers, including dentist, eye doctor, and specialists
- Updating the list of medications
- A personal health risk assessment, including a hearing evaluation and nutritional evaluation
- Safety evaluation
- Any advice or referral services that may help detect potential health risks, such as colonoscopy or lab work
- Establishing a schedule for Medicare’s screening and preventive services over the next five to 10 years
Medicare covers this screening once a year, so there are no out-of-pocket expenses for your appointment.

Community Programs & Outreach
Our primary care team is dedicated to making sure that everyone has access to care and education. We have wellness nurses who speak at long-term care facilities, assisted living centers, and senior health fairs about senior health and well-being.
We also volunteer at community events, like the county fair, to offer blood pressure testing to anyone in the community, not just our patients.
