All reasonable efforts are required to pay your bill. Any unpaid balances, including all applicable co-payments, co-insurance, deductibles and any non-covered services are the responsibility of the patient and must be paid upon receipt of the statement. Without your assistance, your bill could go into extraordinary collection efforts, which includes, reporting to credit bureaus, garnishments, filing a lien or lawsuit, etc. For your convenience we accept a variety of payment methods:
- Cash, check, or money order
- Electronic Funds Transfer from checking or savings
- American Express
Short term payment plans or a bank loan may be available for larger balances. Any payment plan other than payment in full must be agreed to by our office before being considered accepted.
Financial assistance may be available for patients who do not have the ability to pay. Completion of a financial statement and supporting document are required to determine eligibility.
