Patients who are uninsured or fail to provide us with adequate billing information including proper managed care authorization referrals, are responsible for the total payment of their bill.
Any payment arrangements, other than payment in full must be agreed to by our office before being considered accepted.
Financial assistance may be available for patients who do not have the ability to pay. Completion of a financial statement and supporting document are required to determine eligibility.
