Ambulatory Care Management

Ambulatory Care Management is performed on an outpatient basis, without admission to a hospital or other facility. Stormont Vail Health’s Ambulatory Care Management team is designed to assist a patient and his or her support system, helping provide timely, cost-effective, and coordinated care by focusing on preventive services, educating on chronic conditions, and helping with nonmedical barriers. This helps improve patient health status, enhance coordination of care, and eliminate the duplication of services, in hopes to reduce the need for expensive hospital visits.

RN Care Managers and/or Social Workers are caring for patients in various settings to include:

  • All Primary Care Clinics
  • Heart Failure Clinic
  • Endocrinology
  • Post-Acute Care
  • Behavioral Health

All primary care offices throughout the region have social workers and care managers.

Chronic Care Management

Stormont Vail has partnered with Wellbox to offer a chronic care management (CCM) program for patients living with two or more chronic conditions. People who live with persistent health concerns or chronic illnesses often have more complex health requirements. Implementing a coordinated care model like Wellbox can contribute to better overall health and wellness for such patients.

Dedicated Wellbox team members contact patients monthly and focus on disease and medication education, overall wellness, and any questions that may come up between scheduled appointments.
