Heart Failure Care

Heart failure is a condition in which the heart can’t pump enough blood to support our body. When heart failure occurs, it can cause blood and fluid to back up into the lungs, edema (buildup of fluid in the feet, ankles, and legs), tiredness, dizziness, or shortness of breath.

How To Live Well With Heart Failure

Take your medications as directed

Follow your prescribed diet

Limit fluid intake

Weigh yourself daily

Exercise regularly as advised by your care team

Watch for symptoms and contact your provider if there are changes

Don’t miss your scheduled appointments, as they may be frequent at first

Understanding Your Heart Zones

Do a daily self-check. Call your provider if you are in the yellow zone. Waiting until the red zone may lead to hospital admission.
