Stormont Vail Health Preferred Lab Benefit
Stormont Vail Health has negotiated laboratory rates for the State Employee Health Plan (SEHP). At time of service, health plan members must request the Preferred Laboratory Benefit and show an identification card to verify current enrollment in the SEHP.
Laboratory locations and hours of operation are listed on the flyer. Members with a Stormont Vail primary care physician may have blood drawn at their doctor’s office. Laboratory testing results will be reported to the ordering physician.

Our Plans
Plan A
Plan A covers preferred laboratory benefits at 100% with no deductibles, copayment or coinsurance required. Members may have a copayment for an office visit or other services performed as part of the visit.
Plans C, J and N
Preferred laboratory benefits under Plans C, J and N are subject to an annual deductible. After the annual deductible has been met, preferred laboratory benefits are covered at 100% percent.
