Stormont Vail Health and Cotton O’Neil Preferred Lab Benefit

Stormont Vail Health and Cotton O’Neil have negotiated laboratory rates for the State Employee Health Plan (SEHP). At time of service, health plan members must request the Preferred Laboratory Benefit and show an identification card to verify current enrollment in the SEHP.

Laboratory locations and hours of operation are listed on this flyer. Members with a Cotton O’Neil primary care physician may have blood drawn at their doctor’s office. Laboratory testing results will be reported to the ordering physician
