Stormont Vail Health and Midland Care PACE Partner to Expand Cardiac Care Access for Older Adults

Stormont Vail Health and Midland Care’s Program of All-Inclusive Care for the Elderly (PACE) have formed a partnership that expands access to specialized cardiology services for older adults served by Midland Care PACE in the Topeka and Shawnee County area. The collaboration will deliver on-site cardiology care at the Midland Care PACE Topeka center, reducing transportation barriers and helping participants receive timely, coordinated care.

“The PACE program serves an older adult population who often face many barriers to receiving high quality healthcare. The most common diagnoses we manage involve the heart and vascular system, and we are excited to expand our partnership with Stormont Vail to provide access to cardiology care directly within our Topeka clinic,” said Dr. Erin Locke, Medical Director of the Midland Care PACE program. “We believe this will improve access, streamline care and improve health outcomes for our patients.”

Under the partnership, Stormont Vail cardiology providers will collaborate with the Midland Care PACE team to conduct consultations and follow-up visits, coordinate medication management, and streamline referrals for advanced diagnostics or procedures when needed. By integrating specialty care into the PACE model, the organizations aim to shorten time-to-treatment, reduce avoidable hospital visits, and help participants manage chronic cardiovascular conditions with personalized, team‑based care.

“Older adults do best when high-quality specialty care meets them where they are. Bringing cardiology services into the PACE clinic allows us to listen, examine, and act without the friction of travel and scheduling hurdles—ultimately improving outcomes and experience for patients and their families,” said Chris Tollefson Stormont Vail Advanced Practice Provider (APP) Manager, Cardiac and GI Services.
